How to use

STEP1) Find a [Case number] similar to your name structure from the [Name samples] in the table below.

*Refer to…If it start with the last name: Case 1~9.The first name: cases 10~16.

STEP2) Refer to the input pattern listed in [Name (last name)] and [Name(First name)] of [Case number] you found STEP1,Please type in the application form with your name.

*Please select the option if marked "or" in the table only one is allowed.

STEP3) The name entered in [Name (last name)] and [Name (first name)] will be automatically inserted.

*Please correct the name exactly as it appears on the residence card if it not same as it.

• Last Name／氏名（姓）
  • Type in your Last name in English.
  • Please type in only one if your Last name composed of some parts or having some names.

• First Name／氏名（名）
  • Type in your First name in English.
  • Do no type in the middle name on these sections.
  • Please type in only one if your First name composed of some parts or having some names.

• Name／氏名（フルネーム）
  • The characters entered in Name (Last Name)/Name (First Name) will be automatically entered. In the following cases, please correct the residence card so that it is exactly as written.
    1. If the full name and order listed on the residence card are different.
    2. If part of the full name is missing.
    3. Don't forget add the middle name if it written on your residence card.

• Last Name katakana／カナ氏名（姓）
  • Please enter the "kana reading" you entered for your name (last name).

• First Name katakana／カナ氏名（名）
  • Please enter the "kana reading" you entered for your name (first name).

• Name katakana／カナ氏名（フルネーム）
  • The characters entered in Last Name katakana/First Name katakana will be automatically entered. In the following cases, please correct the residence card so that it is exactly as written.
    1. If the full name and order listed on the residence card are different.
    2. If part of the full name is missing.
    3. Don't forget add the middle name if it written on your residence card.
